Alder Hey Paediatric to Adult Audiology Transition Leaflet PIAG 0020 (237kB)

When you reach 18 we will no longer provide care for your hearing and hearing devices at Alder Hey, you will be transitioned to your local adult audiology service who will continue your care.
This information has been put together so you have all the relevant information you will need to help you as you transition to adult services with this process.
The Transition process, what will happen next?
Around your 16th birthday when you attend for your annual review the Audiologist will start to talk with you about the transition preparation and what this entails so you are confident and ready to move to the adult service when you are 18 years of age.
Over the next few years the audiologist or Doctor will discuss the cause of your hearing impairment (if known), the details of your hearing impairment and the possible future impact this may have on working and learning. We will discuss which adult audiology department could provide your care.
Once you have turned 18 we will refer you to your chosen adult service and you will receive an appointment with them. A member of the Alder Hey audiology team will be in attendance at your first appointment, to support you.
Until you have been seen by the adult service you will remain under the care of Alder Hey.

Differences between Adult & Paediatric Audiology departments
The main difference when attending the adult service is there will be no routine/annual appointment for hearing tests and hearing aid checks.
It is up to yourself to contact the hospital and arrange an appointment to attend for maintenance of the hearing aid, repairs, new impressions or if you feel your hearing has changed.
You will be informed how to do this and given contact details at your initial appointment at the Adult service.
The Future
Leaving school is a big life change and sometimes problems can arise. Remember you are not on your own. There are many sources of support available, some of which are documented in this leaflet.
Think ahead about what might make life easier if you meet any challenges at home, work or in further education.
Going to University or College?
Contact your local College/University learning support team who will be able to advise and assess you for additional equipment and any support you might need such as Radio aid, FM systems, note taker, lip speaker, sign language interpreter, and communication support worker.
This may help with lectures, small group work, tutorials etc.

Work
Access to Work Adviser
The Access to Work scheme provides support for you and your employer. The scheme is run by JobCentre Plus and provides funding to assist with the extra support costs which may arise because of your needs. Support can include assessment and advice on special aids or adaptations to the workplace such as text phone or amplified phones, vibrating fire alarm pagers, radio aids, etc.
