Change your appointment
To request a change to an appointment, please complete the form on this page – please note, this form can only be used if the appointment you are cancelling is not within the next 2 days, or you are requesting a change to an appointment out of hours when departments are closed.
Please note, completing this form does not automatically cancel an existing appointment or book another one - it sends a request to one of our team to action. If another appointment is required, you will be sent an appointment when available (if this is a second patient cancellation, a clinician will review to confirm if another appointment should be offered).
Alternatively, you can call to change most appointments over the phone by dialing 0151 228 4811 and selecting option 3. Please note, our Booking and Scheduling Department hours have temporarily changed to 08.30 – 5.00 pm Monday – Thursday, Friday 9 – 5.
If your appointment was arranged through the e-referral system (Choose and Book) by your GP or Dentist:
Please call the Appointment Line (TAL) line on 0345 608 8888 - this is a National service.
If your appointment is with EEG, Neurophysiology or EMG department:
Call the department directly on 0151 252 5375.
If your appointment is with The Alder Centre:
Please call 0151 252 5391.
Alternatively, call 0151 228 4811 and press Option 3; to speak to someone regarding an Outpatient Appointment.
