Although we would always encourage you to make your appointment, we understand that it isn’t always possible. That’s why we’ve made it as simple as possible for you to let us know that you need to change your appointment. To do so, please complete the form on this page.
Please note, this form can only be used if the appointment you are cancelling is not within the next two days, or you are requesting a change to an appointment out of hours.
Completing this form does not automatically cancel an existing appointment or book another one, it sends a request to one of our team to action. If another appointment is needed, you will be sent one when available. If this is a second appointment cancellation, a clinician will review it to confirm if another one should be offered.
You can also call to change most appointments over the phone by dialling 0151 228 4811 and selecting option 3. Our Booking and Scheduling department hours are 08.30 to 5.00 pm Monday to Friday.
